This form has been updated 8th Period and will be updated on Monday before practice, if there are any questions please call the school nurse on Monday.
To Be Cleared:
- All Paperwork with N next to your name must be submitted and the box to the right will indicated that you are cleared.
- Mr. Gazdek is checking grades and should be updated by Monday.
- If there is an N, next to the box click on the Link and get the paperwork submitted.
Also:
- Media Release form must be submitted to get your picture posted on site
- Emergency Medical Form must be submitted by Monday as well
- Both are in the following packet
